How It Works
How To Request Medical Assistance
After profile completion, use Dashboard to view profile progress, request summaries, and current QR booking state.
Generate your personal patient QR from your account for quick identification during service processing.
Open Assistance Request and select the category that matches your case.
Provide patient details, contact, facility, purpose, and related notes for the request.
Check requirement categories for the selected assistance type before uploading.
Upload clear PDF, JPG, or PNG files (max 5MB each) per requirement category.
Submit only when all required categories have files. Once submitted, uploads are locked.
Monitor progress through Pending, Under Evaluation, Approved, For Release, Released, Rejected, or related final states.
Go to Appointment, pick an ongoing schedule, and generate your booking QR pass for Love Caravan services.
Only one active booking is allowed. Rebooking is limited to 3 attempts within 24 hours, with day and slot conflict checks.
Download your appointment QR pass template, and cancel with reason when needed so your status updates correctly.
Use History to review assistance records, medical records, event registrations, and status outcomes in one timeline.

Important reminder: Maintenance medication will not be available at the Love
Caravan. It will be delivered to your home or can be claimed at health centers. For those not
yet listed as recipients for house-to-house distribution, please coordinate with TCISS
representatives present at the Love Caravan or in every barangay.
